Memory Concerns
If you are worried or concerned about your memory, you are able to be seen in your GP practice for an assessment. You must first book an appointment with the nurse, which you can do by clicking this link. Once you have had your memory assessment, you will be booked an appointment with the GP to follow up on your results. We suggest you come to both appointments accompanied by someone who knows you well, and you feel comfortable attending. The GP can then make a decision whether you should have a blood test at the practice or if they want to refer you for a CT scan, or to secondary care. We can then discuss any result with you that come from secondary care or scans.

Support from our Social Prescriber:
Our social prescriber can provide support to patients and families. You can book a appointment with Tracey our SP using the link – Book Here!
Memory Lane Cafe:
Our community link worker, Caroline, currently supports the following local cafe, which offer a Dementia Cafe every 2nd and 4th Monday of the month. If you are interested in joining, or know someone who may be interested in attending, please give Caroline a call on 07816074387.

Dementia Connect in Warwickshire
The service can support anyone living with dementia or caring for someone with dementia. The service provides personalised advice and practical and emotional support, helping you and your loved ones at all stages of dementia.
