The Summary Care Record (SCR) is a new central NHS computer system. It is an electronic record that holds important information about the medicines you take, any allergies you have, and any adverse reactions to medicines you have experienced.

Why do I need a Summary Care Record?

Having your information stored in one place makes it easier for healthcare staff to provide the right care, especially in emergencies or when your GP practice is closed.

This information can be crucial in helping doctors make informed decisions, such as choosing the most appropriate medicines for you.

Who can see it?

Only healthcare staff involved in your care can see your Summary Care Record.

How do I know if I have one?

Check with Your GP Surgery

    • Contact your GP practice and ask if you have an SCR.
    • They can tell you whether your record exists and if it includes basic or additional information.

Use the NHS App or NHS Login

    • Log in to the NHS App or NHS login.
    • Go to your medical records section to check if an SCR is listed.

Contact NHS Digital

    • Call NHS Digital at 0300 303 5678 for SCR inquiries.
    • Email scr.comms@nhs.net to request information.

Do I have to have one?

Participation in the Summary Care Record (SCR) scheme is voluntary. If you choose to opt out, please complete the opt-out form and bring it with you to the surgery. You can find the form at the bottom of this page.
