Sick (fit) notes

Fit notes are issued by doctors after assessing your fitness for work. They are typically provided after the first seven days of sickness absence, as you can self-certify for the initial week.
The doctor may determine that you are either ‘unfit for work’ or ‘may be fit for work’ with specific recommendations for adjustments or changes to help you continue working.
If you have received a sick note from a hospital doctor or GP, you can have it extended by a Clinical Pharmacist or Practice Nurse at our practice. For your initial sick note, you will need to see a GP. However, if you require an extension, there is no need for an appointment—simply contact the practice, and a member of our team will arrange this for you.
We cannot forward date sick-notes, you must ask for a sick note on the day you need it from. We can however back-date.

If you are off work for 7 days or less, you do not need to contact your GP for a sick note.
Your employer may however require you to complete a self-certification form (SC2) which is available from your employer, or you can download it from the HMRC website.
