Separated Parents: Access to Your Child’s Medical Records and Healthcare Information

If you are separated from your child’s other parent, you may have questions about accessing your child’s medical records or healthcare information.

Separation does not automatically remove parental responsibility. However, requests for a child’s medical records or healthcare information need to be considered carefully, taking into account parental responsibility, the child’s age and understanding, confidentiality, safeguarding concerns and any relevant court orders or legal restrictions.

How to make a request

If you would like to request access to your child’s medical records or healthcare information, please contact the surgery and ask for a “Separated Parent – Child Medical Records and Healthcare Access Request Form.”

The form will ask you to provide information such as:

  • Your details and relationship to the child
  • Your child’s details
  • What information or records you are requesting
  • Evidence of parental responsibility, where appropriate
  • Details of any relevant court orders or legal restrictions
  • Any safeguarding or confidentiality concerns that the practice should be aware of

You may be asked to provide appropriate identification and supporting documents, such as a birth certificate, adoption certificate or relevant court order, depending on your circumstances.

What happens next?

Once we receive your completed form and any supporting information, your request will be reviewed in accordance with the relevant confidentiality, information governance, safeguarding and legal requirements.

Please be aware that completing the form does not automatically mean that access will be granted. Some requests may need to be reviewed by a GP, Practice Manager, Information Governance Lead or Safeguarding Lead before a decision is made.

We aim to remain neutral in disagreements between separated parents and will always consider the child’s best interests, wellbeing and confidentiality.
